Passport applications generally require the physical presence of the child and both parents (or a notarised consent from the non-appearing parent), both parents’ passports, the original proof of the child’s birth from the ward or city office (see p.88), an original marriage certificate (if applicable), proof of termination of all prior marriages (if applicable), two identical photos of the child, the application fee – and sometimes evidence of the couple’s physical presence together at conception.
